  • Your date of birth, postcode and relationship status
  • Your GP name and address
  • Records of any discussions we have in either face to face or email sessions

By using this website and submitting your information to us, you agree to us using your personal information in accordance with these terms and conditions. We will not use your personal information for any other purposes.

We will not try to sell you anything and you will not receive unsolicited telephone calls or mail from us or any third parties as a result of personal data being provided to us.

Paper notes from sessions will be stored securely and never contain any personal information linking an individual’s name, address or contact details to them.
